Our Hospice program provides the most compassionate and highest quality end-of-life care through physical, emotional,
and spiritual support.

Hospice provides a multi-dimensional, team approach to caring, comfort, and support for terminally ill patients and their families. Hospice care is available for a wide-range of illnesses including end-stage lung, heart and liver disease, cancer, dementia, stroke and ALS.

As a patient nears the end of life, the RAVNAH hospice team of compassionate, highly trained medical professionals tend to his or her physical and emotional needs. Working with the patient and his or her loved ones, we develop a comprehensive, individualized care plan. We help involve you and your family in the direct care of your loved one, and provide respite and support whenever you need it.

Our staff is also available on call, 24 hours a day, 7 days a week.

Depending upon patient and family needs, RAVNAH's hospice team includes:

  • Nurses
  • Home Health Aides
  • Medical Social Workers
  • Therapists
  • Nutritionists
  • Hospice Volunteers and the Volunteer Services Coordinator
  • Spiritual and Bereavement Counselors

Does your loved one qualify for hospice care?
RAVNAH accepts referrals from physicians, hospital discharge planners, and social workers, as well as from patients and their families.

To be admitted to RAVNAH's Hospice program, a patient must meet the following criteria.

  • The patient has a limited life expectancy,
    confirmed by the patient's primary physician
  • The patient and family both have consented to hospice care
  • The patient and family have agreed to a treatment plan based on
    hospice philosophy, which provides care and comfort only, not cure
  • A primary caregiver has been identified

Your insurance coverage may also have its own requirements and limitations; we will be happy to help you determine coverage eligibility.
