RAVNAH’s Home Care program provides skilled, state-of-the-art intermittent home health care throughout Rutland County and in the Dorset and Rupert areas in Vermont. We work collaboratively with you, your family and your doctor to help you remain safe in your home and to be as independent and active as possible.
Home care is coordinated by a case manager, who creates and individualized plan for your care at home, according to each client's health care needs and physician orders. Our team also includes a full range of home nursing and rehabilitation specialists. In addition, our home health aides provide help with bathing and other activities of daily living. Our social workers and mental health nurses provide counseling and referrals to community services.
Your home care begins with an assessment visit by a registered nurse or physical therapist, who will talk with you to determine your needs and what your plan of care should include. Other members of the team will also visit, as you may need them. Your case manager will continue to be available to answer your questions and adjust your care. Visits will continue until your goals are met.
