Our hospice program provides the most compassionate and highest quality end-of-life care through physical, emotional and spiritual support.

The Hospice program at Rutland Area Visiting Nurse Association & Hospice provides care, comfort, and support for those in the final stages of life and for their families. Hospice care is available for a wide-range of illnesses including end-stage lung, heart and liver disease, cancer, dementia, stroke, Alzheimer’s and ALS.

As a patient nears the end of life, the RAVNAH hospice team of compassionate, highly trained medical professionals, tends to his or her physical and emotional needs. Working with the patient and loved ones, we develop a comprehensive, individualized care plan. Members of the team focus on managing symptoms and pain control to enhance the quality of life for the patient. We involve the patient and family in the patient’s direct care, and provide respite and support whenever it’s needed.

Hospice care is available wherever one chooses to reside – in private homes, nursing homes, assisted living facilities and the hospital.

Our staff is also available on call, 24 hours a day, 7 days a week as needed.

Depending upon patient and family needs, RAVNAH's hospice team includes:

  • Nurses
  • Home Health Aides
  • Medical Social Workers
  • Therapists
  • Nutritionist
  • Hospice Volunteers and the Volunteer Services Coordinator
  • Spiritual and Bereavement Counselors

Does your loved one qualify for hospice care?

Hospice provides skilled care and comfort to adults and children with a terminal illness. To be admitted to RAVNAH's Hospice program, a patient must meet the following criteria:

  • The patient has a limited life expectancy, confirmed by the patient's primary physician
  • The patient and family both have consented to hospice care
  • The patient and family have agreed to a treatment plan based on hospice philosophy, which provides care and comfort only, not cure
  • A primary caregiver has been identified

RAVNAH accepts referrals from physicians, hospital discharge planners, and social workers, as well as from patients and their families.

Who Pays for Hospice?

Most care, supplies, durable medical equipment such as hospital beds, oxygen, and medications related to the terminal diagnosis are covered by Medicare, Medicaid, and most insurance providers. Your insurance coverage may have its own requirements and limitations; we will be happy to help you determine coverage eligibility.

The Hospice program at Rutland Area VNA & Hospice accepts all patients regardless of ability to pay.
