Registered Nurse
(22.5 hrs/wk and per diem)

We're seeking compassionate, team-oriented individuals to provide a full range of professional nursing services to clients of all ages including admission, evaluation and care plan development. Working with a team of home care professionals and paraprofessionals, our nurses strive to achieve the highest level of coordinated care for clients served in Rutland County. The incumbents also participate in ongoing programs to foster intervention, early detection and promotion of community health services.

Candidates must be licensed and currently registered to practice nursing in Vermont, and have a BSN or ADN with two years of applicable experience. Successful candidates will be leaders in their field and possess outstanding critical thinking, communication and assessment skills. Candidates with high-tech and medical/surgical experience are preferred.

Community and Occupational Health Registered Nurse (per diem)

Physical Therapy Assistant (full-time)

We’re looking for an enthusiastic, team-oriented individual to assist in providing physical therapy in the home environment to our adult and geriatric population. Successful candidates will treat, educate and implement established plans of care for clients under the supervision of a licensed physical therapist and in accordance with physician orders.

Candidates will work with a team of therapists, nurses, medical social workers, nutritionists and home healthcare aides to achieve the highest level of coordinated care. Candidates must be certified as a physical therapy assistant in Vermont with 2 years of clinical experience. The successful candidate will possess excellent interpersonal, leadership and organization skills.

Medical Social Worker (full-time)

We’re looking for a dynamic, caring and collaborative individual to assist the physicians, home health professionals, community resource providers and the clients and their families in understanding the psychosocial factors related to the health and medical care of the clients served throughout Rutland County.


The successful individual will assess the clients’ and their families’ capacity and potential to cope with daily living, identify barriers to obtaining a positive medical outcome, and provide solutions in an interdisciplinary approach to help clients achieve their maximum medical outcome. This position is also accountable for preparing accurate and complete clinical notes, billing information as well as other administrative reports.

Candidates must have a bachelor’s degree in social work, psychology,
sociology or other related field with 1 to 2 years of experience in a home health, hospital or mental health setting. The successful candidate will possess excellent communication, networking, critical thinking and assessment skills.

Psychiatric Nurse (15-20 hrs/wk)

As a member of the Clinical Services team, you’ll be responsible for
promoting the health and well-being of clients and their families.
Your role will be to provide in-home supportive counseling, medication
management, medical and ADL assessments, mental status exams and
psychosocial adjustment assessments. Candidates will work with a team
of nurses, medical social workers, therapists, nutritionists and home
healthcare aides to achieve the highest level of coordinated care.

Candidates must be licensed and currently registered to practice nursing
in Vermont and have 2 years of experience in an active treatment unit
of a psychiatric or mental health hospital or outpatient clinic. ANA
Psychiatric Certificate is preferred. Excellent interpersonal, leadership and
organization skills are required.

Private Duty Caregiver for CarePlus (per diem)

Bring your heart to work and make a difference in someone's life. Join our team as a caregiver for CarePlus, RAVNAH's in-home care program.

We’re looking for energetic and caring individuals to provide personal care and companion services to clients of all ages, in the comfort of home. We offer flexible schedules including days, evenings, overnights and weekends, competitive wages, comprehensive training and support, and a respectful workplace.

Candidates must have a high school diploma or GED. Excellent communication, interpersonal and organization skills are a must. Experience as a caregiver or licensure with the State of Vermont as an LNA is helpful.

Under the direction of clinical supervisors, incumbents provide the direct care and supportive services that enable our clients to remain independent and living in their homes. Personal care services include meal preparation, grocery shopping, light housekeeping, assistance with personal hygiene, and client transfer and ambulation.
