We're seeking compassionate, team-oriented individuals to provide a full range of professional nursing services to clients of all ages including admission, evaluation and care plan development. Working with a team of home care professionals and paraprofessionals, our nurses strive to achieve the highest level of coordinated care for clients served in Rutland County. The incumbents also participate in ongoing programs to foster intervention, early detection and promotion of community health services. Candidates must be licensed and currently registered to practice nursing in Vermont, and have a BSN or ADN with two years of applicable experience. Successful candidates will be leaders in their field and possess outstanding critical thinking, communication and assessment skills. Candidates with high-tech and medical/surgical experience are preferred. We are seeking team-oriented individuals who are interested in joining our dedicated team of RNs to help us provide flu clinics, foot clinics, and cholesterol screenings for the public. In this exciting position, you will have an opportunity to provide needed community services a wide range of residents who rely on the Rutland Area VNA and Hospice for high quality service. Physical Therapy Assistant (full-time) We’re looking for an enthusiastic, team-oriented individual to assist in providing physical therapy in the home environment to our adult and geriatric population. Successful candidates will treat, educate and implement established plans of care for clients under the supervision of a licensed physical therapist and in accordance with physician orders. Candidates will work with a team of therapists, nurses, medical social workers, nutritionists and home healthcare aides to achieve the highest level of coordinated care. Candidates must be certified as a physical therapy assistant in Vermont with 2 years of clinical experience. The successful candidate will possess excellent interpersonal, leadership and organization skills. Medical Social Worker (full-time) We’re looking for a dynamic, caring and collaborative individual to assist the physicians, home health professionals, community resource providers and the clients and their families in understanding the psychosocial factors related to the health and medical care of the clients served throughout Rutland County.
Psychiatric Nurse (15-20 hrs/wk) As a member of the Clinical Services team, you’ll be responsible for Private Duty Caregiver for CarePlus (per diem) We’re looking for energetic and caring individuals to provide personal care and companion services to clients of all ages, in the comfort of home. We offer flexible schedules including days, evenings, overnights and weekends, competitive wages, comprehensive training and support, and a respectful workplace. Candidates must have a high school diploma or GED. Excellent communication, interpersonal and organization skills are a must. Experience as a caregiver or licensure with the State of Vermont as an LNA is helpful.
