Clinical Manager - Hospice Program (full-time)
As Clinical Manager of the hospice team, responsibilties include day-to-day management of clinical staff, as well as patient visits and case management as needed. Excellent communication and leadership skills required, as well as ability to work with patients and families facing end-of-life issues.
Candidates must have a BSN, five years of clinical experience, and two years in a supervisory position. Knowledge of pain and symptom management also required.

Registered Nurse (full-time, per diem, and part-time evenings)
We're seeking compassionate, team-oriented individuals to provide a full range of professional nursing services to clients of all ages including admission, evaluation and care plan development. Working with a team of home care professionals and paraprofessionals, our nurses strive to achieve the highest level of coordinated care for clients served in Rutland County, Dorset and Rupert.
Candidates must be licensed and currently registered to practice in Vermont, and have a BSN or ADN. One year of prior clinical experience preffered. On-call weeknights and weekends required.

Psychiatric Nurse (per-diem)
As a member of the clinical services team, you'll be responsible for promoting the health and well-being of clients. Your role will be to provide in-home supportive counseling, medical and ADL assessments, and mental status and psychosocial adjustment assessments. Candidates will work with a team of nurses, medical social workers, therapists, nutritionists, and health care aides to achieve the highest level coordinated care.
Candidates must be licensed and currently registered to practice in Vermont, and have prior experience in psychiatric nursing.

We’re looking for an enthusiastic, team-oriented, community minded physical therapist to join our team of home health care providers to serve our adult and geriatric clients.
Our physical therapists provide on-going case management, education, and guidance to our dedicated team of home healthcare aides, clients and families. Candidates will work with a team of nurses, medical social workers, nutritionists, and homecare aides to provide the highest quality coordinated home health care.

Personal Care Attendants and Licensed Nursing Assistants for the CarePlus Private In-Home Care Program (per diem)
Bring your heart to work and make a difference in someone's life. Join our team as a caregiver for CarePlus, RAVNAH's in-home care program.
We’re looking for energetic and caring individuals to provide personal care and companion services to clients of all ages, in the comfort of home. We offer flexible schedules including days, evenings, overnights and weekends, competitive wages, comprehensive training and support, and a respectful workplace.
Candidates must have a high school diploma or GED. Excellent communication, interpersonal and organizational skills are a must. Experience as a caregiver or licensure with the State of Vermont as an LNA is helpful.
