How to Arrange for Home Care Services

Arranging Home Care Services is Easy.

It can be difficult to know where to turn when you or a loved one needs help with health care issues at home. Rutland Area Visiting Nurse Association & Hospice is here to help and answer all your questions.

We have nurses specially trained to discuss your options, answer questions, arrange services, and clarify insurance coverage. Our goal is to work with you, your physicians and other health care providers to deliver coordinated care, regardless of the simplicity or complexity of  your needs.

Referrals for RAVNAH home health and hospice care come from a variety of sources including:

  • hospitals
  • discharge planners physicians,
  • community health and social service agencies
  • clients and their family members

Our nurses are here to help you 24 hours a day, seven days a week and we can often begin home care services within 24 hours of receiving a referral. If needed, a RAVNAH nurse will make a no-charge evaluation visit to determine if home health services are necessary and how they might be useful.  

Payment for home care comes from many sources, including Medicare, Medicaid, private insurance, the United Way of Rutland County, grants, city and town appropriations, and donations from individuals, companies and foundations.

When a client has limited or no insurance, and the service is medically necessary, RAVNAH uses a sliding fee scale based on the patient’s ability to pay.
